This registration session will allow you to complete contact information and sign up for participation. Please read through each page carefully to ensure that all information is provided accurately. 

PLEASE DO NOT REGISTER UNLESS YOU HAVE SPOKEN TO THE HEAD COACH AND HAVE BEEN INVITED TO JOIN THE TEAM.

ALL PLAYERS, LEGACY AND NEW, need to have the following ready to upload when registering: 

• A photo of your child from the shoulders up standing in front of a neutral wall (not larger than 2MB).  Please make sure you take an updated photo of your child as this picture will be on their player card and is what the referee is looking at to make sure the correct players are on the field.
• a clear picture of your child's birth certificate (not larger than 2MB). It CANNOT be a pdf it must be a jpg.
• Medical insurance card to complete the medical release form.
